Abstract:
- Background: Keloids represent chronic fibroproliferative disorders of the skin which is characterized by excessive deposition of extracellular components especially collagen type 1, fibronectin and elastin. The exact aetiology of keloid formation is undefined but its development rests on genetic and environmental factors.
- Aim: to investigate NEDD4 (rs8032158) gene polymorphism in a sample of Egyptian patients having keloid.
- Patients and methods: The current case-control study was done on 80 subjects; 50 keloid patients and 30 ages and sex matched apparently healthy controls. All subjects underwent full history, weight and length measuring to calculate BMI, dermatological examination and analysis of NEDD4 rs8032158 (T/C) gene polymorphism using Real Time-PCR. Results: rs8032158 CC genotype was significantly observed in keloid patients and increased the risk for keloid development by about 4 times (p=0.038, OR=4.20) and was significantly associated with severe form of keloid (p<0.001). Moreover, C allele significantly increased the risk for keloid development by about 2 folds (P=0.028, OR =2.08). Conclusion: NEDD4 (rs8032158) gene polymorphism could participate in keloid formation in Egyptian population. NEDD4 (rs8032158) CC genotype may have a role in keloid severity.Allelic discrimination plot of (rs8032158) gene polymorphism in both cases and controls (TT genotypes ( red dots), TC genotypes (green dots) and CC genotypes (blue dots)).

Abstract:
Cutaneous warts, caused by HPV, are often symptomatic, cosmetically disabling and difficult to treat due to its propensity for recurrence. Intralesional immunotherapy has been found to be efficacious in treatment of cutaneous warts in recent studies. A total of 90 patients were included in the study. They were randomly divided into 3 groups of 30 patients each. Group 1 patients were given 0.1 ml of Mw vaccine, Group 2 patients were given 0.2 ml to 0.5 ml of intralesional injection of vitamin D3 (60,000 IU, 15mg/ml) and Group 3 patients were given 10 TU/ 0.1 ml of intralesional injection of tuberculin PPD at 3 weeks intervals, until complete clearance of warts or maximum of 4 injections. Follow up was done every 3 weeks for 12 weeks. At the end of the study, Complete clearance was seen in 27 (90%), 25 (83.33%) and 23 (76.66%) patients; partial response was seen in 3 (10%), 3 (10%) and 4 (13.33%) patients; no response was seen in 0 (0%), 2 (6.66%) and 3 (10%) patients of Group 1 and 2 respectively. There were no serious adverse events or recurrence at 24 weeks follow up in both groups. Immunotherapy is quite effective and safe in treatment of cutaneous warts. Intralesional Mw and Vitamin D3 shows comparable results in while Intralesional Mw is found more efficacious than Tuberculin PPD..

Abstract:
Vitiligo is associated with huge social stigma in Asain skin. The desire of novel molecules has been since long in the management of vitiligo. Our aim in the study was to evaluate therapeutic efficacy and safety of dermabrasion with PGE2 analogue (Dinoprostone gel), 5-Flourouracil (5%) cream and Tacrolimus 0.1% cream in localised stable vitiligo
Methodology & Theoretical Orientation: 60 patients of localised stable vitiligo were taken which were randomly divided into 3 groups. Dermabrasion with Dinoprostone gel, 5-Flourouracil and Tacrolimus 0.1% ointment was done in group 1, 2, and 3 respectively. The respective topical agents were instructed to be applied twice daily till 12 weeks. Follow up was done by VASI, DLQI and photographically four weekly till 36 weeks.
Findings: Repigmentation started within 4 weeks in all 3 groups. Excellent response (>75% repigmentation ) was seen in 43%,40% and 54% of group1,2 and 3 patients respectively. Good response(>50% respigmenatation) was observed in 34%,26%, 33%, of patients of group1,2 and 3 respectively. 25-50% repigmentation was seen in 4%,8%,10% in group1,2 and 3 aptients respectively. No significant side effects or reoccurence were found in the study period.
Conclusions and significance: Dermabrasion when combined with Dinoprostone gel, 5-Flourouracil (5%), or tacrolimus 0.1% ointment showed similar efficacy and side effect profile in all 3 groups with slight better response with tacrolimus ointment. This study adds novel molecule dinoprostone to the already existing basket for vitiligo mangement

Introduction:
Partner notification is an important key part of management for sexually transmitted infections. It is essential to reduce chronic, recurrent infections and transmission. But among Bangladeshi population, the concept of partner management is extremely low. To minimize the problem, recent perceptions of partner notification among people are needed to be evaluated and explored further.
Objective:
The aim of the study is to evaluate the awareness of partner notification among male patients attended in a tertiary level hospital and cumulate the information to improve partner notification awareness programs
Methods & Materials:It was a cross sectional study, being conducted among 67 male patients aged between 22-50 years, educational status between elementary to graduate, attended in Skin & Venereal Disease outdoor patient department of Dhaka Medical College Hospital, Dhaka ,Bangladesh from July,2019 to September,2019. Patients were selected specifically suffering from urethritis following having sexual intercourse with other than their legal partners without condoms. Participants were interviewed separately with a pre-formed set questionnaire. Findings were analyzed to comprehend the awareness of partner notification
Result:
All participants (67) were married. All had one or more sex partners other than wife. 61 (91.04%) were suffering from recurrent STI, 6 were suffering from acute STI though none was found HIV positive.
Among all participants, 44 (65.67%) were completely unaware of partner notification. 13 (19.40%) were aware, but did not notify for the fear of familial disharmony, 7(10.44%) intended to notify but it was too late and meanwhile transmitted the infection, 3(4%) considered the notification worthless.
Conclusion:Considering the limitations, this study will provide some direction to research for the improvisation of awareness of partner notification among the people of Bangladesh.

Abstract:
Introduction Amongst the different modalities for the treatment of warts, not a single modality is fully effective. While treating warts, the possibility of spontaneous regression must also be kept in mind. The aim of treating warts is to induce long term immunity without recurrences.
Aims
To study the efficacy and safety of intralesional bleomycin for the treatment of twenty patients with periungual and palmoplantar warts.
Methods We conducted a prospective study of twenty patients of resistant periungual and palmoplantar warts between the age group of 15 – 50 years. Three injections of bleomycin were given at an interval of 2 weeks for a maximum of 3 injections(or less if the resolution occurred earlier).
Results After three sessions, complete clearance of palmoplantar warts was 71.42% and that of periungual warts was 90.90%. lesions were seen in 82% patients and incomplete resolution was seen in 15% patients. Bleomycin has antibacterial, antiviral and cytotoxic activity. Commonest side effect after bleomycin injection was pain seen in 40%(8) patients followed by pigmentary change and raynauds phenomenon seen in 5%(1) patient each.
Discussion
Bleomycin is an effective and promising treatment of recalcitrant warts especially in palmoplantar and periungual

Abstract:
Retinoids refer to all natural and synthetic products having a structure or biological activitiessimilar to vitamin A, which helps to modulate the function of homeostasis, metabolism, epithelialgrowth and immune regulation as well as inflammation via retinoid receptors. Recently, there hasbeen a growing interest in retinoids in the management of UV-related skin conditions, includingskin cancer, solar ageing as well as dyspigmentation, which cause a huge dermatological andpsychological burden on patients. However, scientific evidence and a standard guideline of retinoidson these conditions are still limited. Skin cancer is caused by accumulative DNA damage by UVRalong with an impaired DNA repair mechanisms. Retinoids seem to have a chemopreventive effectby modulating the repair processes and programmed cell death. Acitretin appears to be effective inthe chemoprevention of SCC among OTRs but due to the side effects of systemic retinoids, theapplication should be limited to high-risk populations and the rebound effect may occur afterdiscontinuing medication. UVR also causes skin ageing that leads to a structural and functionaldeterioration of skin. Wrinkling, mottled hyperpigmentation and solar lentigo can be improved bylong-term use of topical retinoids, such as topical tretinoin an adapalene. The efficacy seems to becorrelated with the strength of retinoids along with their side effects, which can gradually subsideovertime. Thus, low concentration of retinoids along with an emollient as well as a sunscreenapplication should be introduced during the initiating period to avoid drug interruption ordiscontinuation. Due to several limitations of clinical studies, the efficacy of cosmeceutical productson skin ageing is still controversial. Besides, long-term use of topical tretinoin may improvemelasma but a treatment combination along with topical retinoids is suggested to yield asatisfactory result with minimal side effects.(OTR- Organ transplant recipient, SCC- Squamous cell carcinoma, UVR-Ultraviolet radiation)

Abstract:
Grapes (Vitis vinifera) are one of the most widely-grown fruit crops in the world. Approximately 75% of the whole grape production is utilized into winemaking, while 25% of the original grape weight is grape pomace (skin, seeds and stalks), which is a by-product. The oil-soluble fraction of grape pomace is rich in phenolic compounds including flavonoids, carotenoids, phenolic acids, tannins, and stilbenes, high unsaturated fatty acids, vitamins (E, A and C) and phytosterols with economic importance to cosmetic, food and pharmaceutical industry. The objective of this study was to evaluate the cosmetic efficacy on skin balance, normalization, hydration and elasticity and on wrinkles prevention of dermocosmetic formulations containing grape seed oil extract on young and mature skin using in vitro and in vivo techniques.In vitro studies showed anti-aging effect through enhancement of cell detoxification, hydration, protection of cellular proteins, stabilization of connective tissues from UV-stress inducing hyaluronic acid synthase and stimulating collagen I and III synthesis.In vivo studies displayed wrinkles volume and depth reduction and a barrier protective effect inducing a high hydration of the skin.

Application of laser in drug delivery of cancer tissue: Using a laser and a method for making drugs with nanoparticles, we can use the laser wavelength and appropriate selection of drugs and nanoparticles to target tissues that are used in drug delivery.
